What happened to the practice of medicine that you used to love?
You are tired, overworked, and unhappy
“But I love what I do”
Do you love being tired?
Do you love being overworked?
Do you love being unhappy?
“But I want to help people”
That’s great.
Noble.
Admirable.
But the people that you want to help don’t belong to you.
They belong to an insurance company.
And that’s your problem.

And that’s your problem.
You tell yourself that "your" patients come to you because they want you
But you know, they know, and I know that the only reason they come to you is because the insurance company that owns them decided to let you be listed on the list of doctors that they can see
Do you really think that because you look at them in the eye and hold their hand or console them about X,Y,Z better than the next doc,
that they’re not going to go that doc when the insurance company that owns them decides to take you off the list of doctors that they can see?
You can tell yourself that patients will ignore that list of doctors that they can see.
You can tell yourself the patients will gladly pay cash to be looked in the eye, to be held by the hand, and to be consoled.
You can tell yourself whatever you want, but you know it isn’t true. Not even remotely.
How many times have your heard, I haven’t been here in a while, my insurance changed.
If you needed that cash to pay for your food, pay for your kids or pay for those identical overpriced medications that both you and next doc are going to prescribe, what would you do?

You can tell yourself that you can still help people
And that you’re OK with the reality that the people you’re helping don’t belong to you
And that you're OK that they mostly don’t value your “unique value proposition” to see you when your name is cancelled from the insurance company’s list.
And when you tell yourself that story...
You’re dead
Because now the insurance company owns YOU.
Because if your business is completely dependent upon being on that list
And you know it
And they know it
They can tell you exactly how they want you help people.
And that means cookbook medicine, cheap-first care, and no questions asked.
And if you don’t do what they tell you to do, they will take you off their list.
No list. No patients. No patients. No business.
Do you hear me now?

They want to work you as hard as they can and pay you as little as they can until they burn you out
They want you to practice cheapness-first cookbook medicine, treat your patients like cattle, and keep your mouth shut no questions asked.
Your version of “I want to help people” in their system, the system in which you have chosen to spend the rest of career is to deliver commoditized low quality services like an assembly line worker in a third world factory.
No equity. No gratitude. No respect.
